Need a translation/decoding of plate

Finally!!! I have figured out how to post pictures... Message to the forum master, this was a pain to do, there are many other forums that allow one to upload the image from the desk top... If you can figure out a way to do this without have to  go through 10 steps might make it easier on all of us. Just some constructive criticism...
