Do the W116 cars not come with this bushing from the factory?

That's what I was wondering, as I dont see any bushing there currently. I was surprised to see it in EPC.  Maybe you drive the pin out, drill to fit bushing and put the pin back in.

I may get a couple bushing and give it a try.

Please keep us posted. Them hinges have almost been unobtanium for quite some time. I think, over 3 years  I found 2 or 3 of them only and finally gave up on finding any more. A refurbishment option would be great.
How often doe these hinges sag, btw? Is that a common thing?

I have never seen a w116 door sag.  Many  ford au falcon doors do though.

Best thing about benzes and volvos of the era they were cars with DOORS!

Now that it's been made mention of, I've never seen a W116 hinge wear out, either.  I wish they'd kept the early style door hinges that the early W108/W109 cars used.  They were adjustable!

I'll add my name to the list of those who have never seen a 116 door hinge wear or sag.

The front driver side door hinge on my W116 is worn out. The door sags and moves a up and down, so I definitely need to fix or replace it sometime. The door latch mechanisms, seats, and seatbelts were also worn out. My car has 343,000 miles on the odometer, though. I greased the door hinges and replaced the latches and strikers.

I'd say it would depend greatly on climate/operating conditions. I've not seen a worn one either. Ideally they should be greased every now and then.

I suppose it would also depend on if the door hinges were greased regularly or not.  I've always been shocked at people not knowing that all Mercedes door hinges since the 1950's have a place to stick a grease gun into to grease.  Unfortunately, greasing door hinges fall into the same category that glass, door, and tail light seals fall under. People don't realize that those are wear/service items, and must be replaced regularly, or as needed.  Door hinges should be greased regularly, too!

Yesterday a friend and I tried getting one hinge apart using a press.  first attempt was met with failure as we supported the hinge toward the outside, instead of in the area directly around the pin.  we bent the whole hinge and pin pretty bad and ruined it.  after I left he worked on and found that properly supported you can get the pin out.

He also reported that the pin was more worn out than the bushing.  I have not seen the parts yet, but he mentioned the pin wear was not all the way around, so perhaps you could rotate it 180 degrees when you put it back together with new bushings.
